The first step is completing the attached personal financial statement.

The second step depends on the type of business you have:

• For Sole Proprietors, we need 3 years Personal Tax Returns and Fictitious Name Filing
• For Partnerships, we need 3 years Personal Tax Returns, 3 years Business Tax Returns, and Fictitious Name Filing or Other State Filing
• For Limited Liability Companies, we need 3 years Personal Tax Returns, 3 years Business Tax Returns , Articles of Organization and Operating Agreement
• For S Corporations and C corporations, we need 3 years Personal Tax Returns, 3 years Business Tax Returns, and Articles of Incorporation
